Public Health Nurse 

The Hopewell Township Public Health Nurse provides community health services by integrating a clinical understanding of health & wellness with epidemiological methods to implement an appropriate public health response. Core activities include following-up on communicable disease reports, ensuring access to vaccinations, and conducting immunization audits at schools and childcare centers. Program goals include prevention of illness, injury or disability, and advancing the health of the population in partnership with the community.

Family Connects 

Mercer County partnership with Family Connects provides free at-home nurse visits for parents who live in Mercer County and give birth at Capital Health, regardless of household income or health insurance status. ScreenNJ aims to increase cancer awareness and share information about cancer screening, treatment, research and survivorship services, as well as helping organizations initiate or expand outcomes oriented, evidence-based cancer screening, education and outreach services.
